脾虚湿盛型特应性皮炎患者部分免疫学特点研究

摘要:

[目的]探讨脾虚湿盛型特应性皮炎(AtopicDermatitis,AD)患者部分免疫学特点。[方法]采用ELISA方法检测门诊61例中医辨证属于特应性皮炎(AD)患者和20例健康人的血清总免疫球蛋白E(IgE),采用流式细胞仪检测外周血T细胞亚群及T辅助细胞(Th)Th₁T/h2状态,利用RT-PCR方法检测外周血单个核细胞干扰素γ(IFN-γ)及白介素-4(IL-4)mRNA表达。[结果]AD患者血清总IgE水平显著高于健康对照组(P<0.01),且外周血CD₃⁺T、CD₄⁺T百分比显著高于健康对照组(P<0.01或P<0.05),而CD₈⁺T百分比低于对照组(P<0.05),患者外周血Th₁细胞百分比低于健康对照组(P<0.01),而Th₂百分比高于健康对照组(P<0.01),IL-4mRNA表达显著高于对照组(P<0.01)。[结论]血清总IgE的升高,T淋巴细胞功能的紊乱,Th₁T/h2的失衡构成了脾虚湿盛型AD患者的部分免疫学特点。

Abstract:

[Objective] To investigate the immune character in patients with atopic dermatitis in damp abundance due to splenic asthenia syndrome AD.[Methods] Sixty.one patients with atopic dermatitis were enrolled from out.patient clinic.Twenty healthy persons were as control.The level of serum total IgE was determined by ELISA The T cell subgroup of peripheral blood and Taccessory cell Th₁/Th₂ were determined by flow cytometry.The IFN.γ and IL.4 mRNA expression in eripheral blood mononuclear cells were determined by RT.PCR.[Result] The level of serum total IgE in patients with atopic dermatitis was higher than that in control group P<0.01.The percentage of CD₃⁺T and CD₄⁺T wase significantly higher than that in control group P<0.01 and P<0.05.But the percentage of CD₈⁺T was remarkablely lower than that in control group P<0.05.The percentage of Th₁ cell in the peripheral blood was significantly lower than that in control group P<0.01.The percentage of Th₂ cell in the peripheral blood was significantly higher than that in control group P<0.01.The expression of IL.4 mRNA was significantly higher than that in control group P<0.01.[Conclusion] The high level of serum total IgE,the disorder of T cell function and the imbalance of Th₁/Th₂ were the immune character in patients with atopic dermatitis in damp abundance due to spleen asthenia syndrome.
