眼肌直刺为主治疗外伤性眼外肌麻痹临床研究

摘要:

[目的] 观察眼肌直刺为主治疗外伤性眼外肌麻痹的临床疗效,探索治疗机制。[方法] 对照组:常规肌肉注射维生素B1及B12,口服血管扩张剂配合眼周穴位针刺49例52只眼。治疗组:眼肌直刺为主配合眼周穴位针刺58例60只眼。治疗1～6个疗程并随访观察1～6个月,比较两组临床疗效。[结果] 对照组治愈35例36只眼,治愈率为69.23%,好转3例3只眼,显效3例3只眼,无效8例10只眼;治疗组治愈51例51只眼,治愈率为85.00%,好转4例4只眼,显效2例3只眼,无效1例2只眼。治愈病例随访观察1～6个月均未见复发,两组疗效比较有显著性差异(χ²=7.3595,P<0.01)。两组比较在疗程上有显著性差异,治疗组疗程短于对照组。[结论] 眼肌直刺为主治疗外伤性眼外肌麻痹疗程短,见效快,治愈率高。其治疗机制可能为刺激了肌肉组织和相应神经分支,直接兴奋病理状态下的麻痹肌,使其收缩力增强,促进了神经肌肉功能的恢复。

Abstract:

[Objective] To observe the clinical effect of direct acupuncture method in treating traumatic external opthalmoplegia and explore the treatment mechanism.[Methods] Forty-nine patients in control group (52 eyes) were treated with injecting vitamin B1, B12, taking vasodilatator agent and acupunture treatment. Fifty-eight patients in treatment group (60 eyes) were treated with direct acupuncture method combined with local acupuncture. These patients were followed up 1～6 months after 1～6 periods of treatment. The clinical effects were compared between these two groups.[Results] The 35 cases (36 eyes) were cured, and 3 cases (3 eyes) were efficacious, and 3 cases (3 eyes) were significant effective, and 8 cases (10 eyes) were ineffective, and the cured rate was 69.23% in the control group. The 51 cases (51 eyes) were cured, 4 cases (4 eyes) were efficacious, and 2 cases (3 eyes) were significant effective, 1 cases (2 eyes) were ineffective, and the cured rate was 85.00% in treatment group. Recurrence did not occur in the two groups during the follow-up survey from 1～6 months. There was a significant difference between the cured rates of two groups(χ²=7.359 5;P<0.01).[Conclusions] The effect of treatment group was quick, and period of treatment was short, the cured rate was high. The mechanism of direct acupuncture method may probably be promoting muscle shrinkage and recovering the action of nerves by stimulating muscle or relevant nervous ramification and exciting pathological paralysis of muscles.
