从脑内微小病变探讨中风“治未病”客观依据的研究

摘要:
[目的]探讨脑内微小病变与中风危险因子的关系及中风发病的相关性,进而探讨脑内微小病变能否作为中风"治未病"的客观依据。[方法]将头颅核磁共振成像(MRI)上的微小病变按照不同表现型和解剖部位分别考察它们与中风危险因子的关联及追踪随访它们与中风发病的关系。[结果]微小病变主要分布在基底节上部,频度最高的是前有孔质的Ⅲ型(73.3%)和基底节上部的Ⅲ型(47.4%)。基底节上部的Ⅲ型与年龄有很大的关联;Ⅱ型与年龄及眩晕相关;Ⅰ型与饮酒习惯相关。皮质下白质的Ⅲ型与消渴相关。基底节上部的微小病变尤其是Ⅱ型与Ⅲ型与中风发病有很大关联。[结论]脑内微小病变与中风有共同的危险因子;与中风发病相关,是中风发病的预知因子;脑内微小病变为中风"治未病"提供了客观依据。

Abstract:
[Objective] To explore the relation between micro-lesion of brain and stroke morbidity and risk factors, and to prove its role as an early warning sign for stroke. [Methods] Micro-lesions of brain were identified by brain MR imaging, separated by their pathological types or anatomical localizations, and their relationships with stroke symptoms and different stroke risk factors were then investigated separately. [Results] Micro-lesion of brain was mainly distributed in the upper part of the basal ganglia, and the type Ⅲ (73.3%) in anterior perforated substance and type Ⅲ (47.4%) in the upper part of the basal ganglia were most frequently observed. The upper part of the basal ganglia type Ⅲ had a close correlation with age; typeⅡ had a correlation with age and vertigo; type I had a correlation with drinking. The upper part of the basal ganglia mircro-lesions, particularly the type Ⅱand type Ⅲ had great relevance with incidence of stroke. [Conclusion] Micro-lesion of brain share common risk factors with stroke, and was also strongly correlated with the onset of stroke. Our study was the first to provide evidences for the characteristic role of mricro-lesion as a warning sign for stroke.
