自拟术后康复汤促进妇科术后恢复的临床观察

摘要:

[目的]评价自拟术后康复汤对促进妇科手术后恢复的疗效。[方法]将100例患者随机分为试验组50例,对照组50例。两组术前基础治疗相同,试验组术后12h即服用术后康复汤浓煎剂50mL,2次/d,共3d。对照组术后继续应用抗生素2d,不服用术后康复汤。观察两组排气、排便时间,术后病率,切口甲级愈合率,术后血红蛋白(Hb)、血浆纤维蛋白原(FIB)、血浆D-D二聚体。[结果]试验组排气时间、排便时间均明显早于对照组;试验组术后病率明显低于对照组;试验组术后Hb明显高于对照组,而FIB、D-D二聚体均明显低于对照组(P<0.05)。[结论]自拟术后康复汤能有效促进肠蠕动、显著降低术后病率、预防术后感染、减少下肢血栓形成,迅速升高血红蛋白,减少预防性抗生素应用的剂量及疗程,降低医疗费用,值得临床推广应用。

Abstract:

[Objective] To evaluate the effect of decoction of post-operation recovery in promoting body recovery after gynecologic operation. [Methods] One hundred cases underwent total or subtotal hysterectomy for uterine myoma or adenomyosis were randomly assigned to 2 groups. They received a same basic treatment. Fifty patients in the trial group were administrated basic treatment plus decoction of post-operation recovery 50 mL, twice a day, for 3 days. Another 50 patients in the control group were administrated with the same antibiotic as the one in basic treatment for 2 days, without giving Chinese medicine. The time of exsufflation and defecation, disease incidence of post-operation, and ratio of healed incision grade A were recorded. Hemoglobin, plasma fibrinogen and Ddipolymer were detected. [Results] The average time of exsufflation and defecation after operation in trial group was significantly earlier than that of control group (P<0.05). There was a dramatically lower disease incidence of post-operation in trial group(10%) compared with 26% in control group(P< 0.05). Hemoglobin in trial group was significantly higher than that of control group,(P<0.05). Plasma fibrinogen and D-dipolymer in trial group was significantly lower than that of control group (P<0.05). [Conclusion] Decoction of post-operation recovery can accelerate the intestinal peristalsis, reduce the disease incidence of post-operation, prevent infection from operation, avoid thrombogenesis. It can also raise hemoglobin quickly, decrease therapeutic course and dosage of antibiotic. So it was deserved to apply in Chinese medicine widely.
