体外高频热疗联合中医辨证治疗气虚毒瘀型晚期非小细胞肺癌的临床研究

摘要:

[目的] 观察体外高频热疗联合消岩汤对气虚毒瘀型晚期非小细胞肺癌(ⅢB期、Ⅳ期)患者生活质量的影响.[方法] 将40例气虚毒瘀型晚期非小细胞肺癌患者,随机分为联合治疗组(接受体外高频热疗联合口服消岩汤治疗)、中药治疗组(单纯接受口服消岩汤治疗),两组均连续治疗60 d.治疗前后评定中医证候、美国东部肿瘤协作组(ECOG)评分、癌症患者生存质量测定量表(EORTCQLQ-LC43)及癌症治疗功能性量表(FACT-L)评分.[结果] 两组间治疗后比较,联合治疗组的中医症候评分提高水平较中药治疗组明显(P < 0.05),联合治疗组在EORTCQLQ-LC43量表中的躯体领域、情感领域的改善程度上较中药治疗组改善明显(P < 0.01),在疼痛改善情况上较中药治疗组明显(P < 0.01).而联合治疗组在FACT-L量表中的躯体状况、社会/家庭情况、功能情况、量表总分均较中药治疗组明显提高(P < 0.01).[结论] 体外高频热疗联合中医药治疗能提高气虚毒瘀型晚期非小细胞肺癌患者中医症状及生存质量,在躯体、情感、疼痛症状的改善程度方面有明显优势.

Abstract:

[Objective] To observe the effect of external high frequency thermotherapy(EHFT) combined with Xiaoyan decoction on quality of life in patients with advanced non small cell(ANSC) lung cancer of Qi deficiency toxicity and stasis type. [Methods] Forty patients with Qi deficiency, toxicity and blood stasis type of ANSC lung cancer were randomly divided into combined treatment group (receiving EHFT combined with oral Xiaoyan decoction), TCM treatment group (received oral Xiaoyan decoction only) for 60 days. The TCM syndromes, including ECOG, EORTCQLQ-LC43 and FACT-L score before and after treatment were evaluated. [Results] After comparison analysis of two groups after treatment, TCM syndrome score of combined treatment group increased more obviously than TCM treatment group (P < 0.05), the improvement of body and affection domain in EORTCQLQ-LC43 scales of combined treatment group was also more obvious than TCM treatment group(P < 0.01), the improvement in pain was more obvious than TCM treatment group(P < 0.01). The physical status, social / family condition, function, scale total scores of FACT-L scale in combined treatment group increased significantly more than TCM treatment group (P < 0.01). [Conclusion] The external high frequency thermotherapy combined with TCM treatment can improve TCM symptoms and the existence quality in patients with advanced non small cell lung cancer of Qi deficiency toxicity and stasis type. It can improve the degree of physical, emotional and pain with obvious advantages.
