| 探讨对社区糖尿病患者实施群组管理的工作模式和实践。回顾分析对社区糖尿病患者实施群组管理的经验,举实例说明其具体的方法和内容,提出目前尚存在的问题和对未来发展的建议。对社区糖尿病患者实施群组管理,需要建立适宜的医疗队伍和患者群组,并从健康体检、专题讲座、“一对一”诊疗、互动活动和考核等多方面开展。群组管理模式适于中国现阶段社区糖尿病的管理,但该模式尚有许多问题需要逐步解决和完善。 |

| To explore the management mode and practice for patients in group in the society area. The experience of group management for patients with diabetes was retrospectively analyzed and an actual example was shown to illustrate this mode. The existing problems and the suggestions for the future work were also presented. It was necessary to establish a medical team and a suitable patient group for the group management mode in society area. This mode included medical examinations, lectures, individual visit, interactive activities and assessments, etc. Group management is an effective way to manage patients with diabetes in the society area in our country. Nevertheless, there are still some problems need to solve and improve. |
