红外热成像评估原发性骨质疏松腰背疼痛的应用研究

摘要:
[目的]探讨医用红外热成像技术在原发性骨质疏松腰背疼痛治疗中的应用价值。[方法]将80例合格受试者随机分为2组,对照组予以钙尔奇D、罗盖全、福善美治疗。观察组在对照组治疗基础上,给予腰背疼痛分布区域相应节段夹脊穴针刺治疗。两组疗程均为4周。分别于治疗前、治疗结束后,测量两组视觉模拟评分法(VAS)评分、压痛指数、红外热成像热图温度均值,比较上述指标组内治疗前后及组间差异。[结果]组内治疗前后比较,两组VAS评分、压痛指数、红外热成像热图温度均值均有改变(P<0.05);组间比较,观察组VAS评分、压痛指数在治疗2周和4周后,均优于对照组(P<0.05);热图温度均值在治疗4周后,组间无统计学差异(P>0.05),但温度均值变化值,观察组优于对照组(P<0.05)。[结论]夹脊穴针刺治疗有助于加快改善原发性骨质疏松患者腰背疼痛症状,且红外热成像热图温度均值对骨质疏松患者慢性腰背疼痛程度有一定提示作用。

Application research of infrared thermography in the evaluation of low back pain from primary osteoporosis

Abstract:
[Objective] To investigate the application value of infrared thermography in the treatment of low back pain from osteoporosis.[Methods] The 80 eligible subjects were randomly divided into two groups. The control group was treated by taking Caltrate D, calcitriol and alendronate sodium orally. The study group, on the basis of the treatment of the control group, was treated with acupuncture at the corresponding jiaji points in the low back pain distribution. The courses of two groups were both 4 weeks. VAS, pressure pain index and mean temperature of thermal map were measured before and after treatment in two groups. The differences were compared within and between the groups. The correlation between VAS and mean temperature of thermal map was analyzed.[Results] The VAS score and the mean temperature of thermal map had significant positive correlation (P<0.05). The dfference of VAS, pressure pain index, mean temperature of thermal map before and after treatment within two groups are all significant (P<0.05). Between two groups, there are significant differences in the VAS score and pressure pain index after both 2 and 4 weeks of treatment (P<0.05); and no significant difference in the mean temperature of thermal map after 4 weeks of treatment (P>0.05), but the difference of the variation of the mean temperature after 4 weeks of treatment is significant (P<0.05).[Conclusion] To some extent, the mean temperature of thermal map can be used as a quantitative indicator to describe the degree of low back pain from osteoporosis. Acupuncture at Jiaji acupoints can help to accelerate the improvement of the symptoms of low back pain from osteoporosis.
