向心性缠绕手指对偏瘫后肩手综合征的康复护理临床疗效观察

摘要:

[目的]观察向心性缠绕手指对偏瘫后肩手综合征的康复护理临床疗效。[方法]纳入82例偏瘫后肩手综合征患者，进行向心性缠绕手指，每日2次，上下午各1次，每周进行手部肿胀程度、运动功能和疼痛的测量。[结果]治疗2 d后76例（92.7%）患者患手皮肤出现皱褶，患者自觉患手肿胀感觉明显减轻，指间关节活动度增加，并且活动时疼痛较前减轻。治疗1周后73例（89.0%）患者患手水肿较治疗前减轻40%；治疗2周后68例（82.9%）患者患手的关节肿胀程度较前减轻80%，关节活动度接近正常，关节活动时无疼痛。治疗2周后患者的Fugl-Meyer量表得分高于治疗前，而疼痛视觉模拟（VAS）评分和肿胀程度低于治疗前，治疗前后比较具有统计学意义（P<0.05）。[结论]向心性缠绕手指是治疗肩手综合征，尤其是手肿胀一种有效的康复护理方法。

Abstract:

[Objective] To observe the rehabilitation nursing clinical efficacy of concentric winding fingers for shoulder-hand syndrome post hemiplegia.[Methods] A total of 82 cases of shoulder-hand syndrome post hemiplegia were included and received nursing of concentric winding fingers, 2 times a day, once in the morning and once in the afternoon. The swelling, motor function and pain of hands were detected every week.[Results] After 2 days of treatment, the hand skin of 76 (92.7%) patients appeared folds, and the patients consciously felt that swelling sensation significantly reduced. Interphalangeal joint range of motion increased, and pain during activity alleviated. After one week of intervention the cylinder measuring showed that hand edema of 73 (89.0%) patients which had reduced 40% than that before treatment; after two weeks of intervention the cylinder measuring showed that hand edema of 68(82.9%) patients which had reduced 80% than that before treatment, and range of motion got close to normal with no pain during joint activities. After two weeks of intervention, patients got higher FMA score than before treatment, while VAS score and the degree of swelling was lower than that before treatment, with statistical significance in comparison between before and after treatment (P<0.05).[Conclusion] Concentric winding finger was an effective method of rehabilitation care for shoulder-hand syndrome, especially swelling hand.
