从发病学看体病相关的新视角

摘要:

现代医学对疾病的发生多从致病因子、宿主、环境3个要素来思考，在发病学上也偏重于单个病种的研究。体病相关是发病学上一个新的视角，考虑到致病因素和个体体质关系，研究“人的病”的同时，更关注“病的人”，体病相关理论得到了文献、流行病调查以及分子生物学系列实证支持。文章结合发病学理论，系统阐明了体质在发病学先天三要素（禀赋、遗传、胎源）中的重要作用；提出疾病发生与转归体质三说即易感说、从化说、土壤说。并基于体病相关的发病学理论，提出“辨体用方，方体相应”的辨体论治思想、阐明“同病异治，异病同治”的本质，建立“辨体-辨病-辨证”诊疗模式。在体病相关理论的指导下，改善体质将是中医防治疾病的新途径，在方药研究方面也将产生新的思路与成果。

Abstract:

Modern medicine treats diseases from three factors:pathogenic factors, host and environment. In the pathogenesis, it also focuses on the study of individual diseases. Constitution-disease relation is a new perspective on pathogenesis. Considering the relationship between pathogenic factors and individual physique, studying human disease, more attention is paid to sick people. The theory of constitution-disease relation has been investigated in literature and epidemiology. And empirical support in the molecular biology series. In this paper, combined with the theory of pathogenesis, the author systematically clarifies the important role of constitution in the congenital three elements of the pathogenesis (endowment, heredity, fetal source); it puts forward the three states of disease occurrence and outcome, namely, the susceptibility theory, the syllabus, and the soil theory. Based on the theory of pathogenesis related to body diseases, the author puts forward the idea of prescriptions based on body constitution,correlation between prescription and constitution, and clarifies the essence of treating the same disease with different therapies, and treating different diseases with the same therapy, and establishes constitution differentiation-disease differentiation-syndrome differentiation model in clinical diagnosis. Under the guidance of the theory of constitution-disease relation, improving physical fitness will be a new way for Chinese medicine to prevent and treat diseases, and new ideas and results will be produced in the research of prescription drugs.
