推拿手法临床试验中盲法实施与评价的方法学思考

摘要:
推拿手法作为中医非药物疗法的重要组成部分,开展高质量临床疗效研究与评价,是推动该疗法现代化与国际化的关键。然而,推拿操作固有的个体化、体感依赖性等特征,为临床试验中盲法的成功实施与评价带来显著挑战,直接影响研究结论的可靠性与证据等级。文章系统梳理推拿手法临床试验中盲法应用的现状、核心难点及方法学进展:深入剖析盲法在控制测量偏倚与实施偏倚中的核心作用,揭示当前领域盲法报告率低、假干预设计科学性不足、评价方法单一且报告标准缺失的严峻现实。为破解这些困境,引入适用于操作性干预的标准化方法学框架(DITTO框架),为假干预方案的系统化设计提供结构化思路;探讨James盲法指数(JBI)与Bang盲法指数(BBI)等量化工具在盲法质量评价中的应用价值与局限,主张建立多角色、多维度评价体系。整合虚拟现实(VR)/增强现实(AR)等现代技术优化盲法设计、开展大规模方法学研究建立推拿特异性标准、强化学术透明化建设。文章认为,唯有通过DITTO框架赋能假干预标准化设计、构建多维度盲法评价体系、推动技术创新与学术规范融合等方法学创新与规范化路径,才能从根本上提升推拿临床研究的科学严谨性,为其中医疗效提供令人信服的高级别证据。

Methodological reflections on the implementation and evaluation of blinding in clinical trials of Tiuna manipulation

Abstract:
As a vital component of non-pharmacological therapies in traditional Chinese medicine,the high-quality clinical research on Tiuna manipulation is crucial for its modernization and international acceptance. However,the inherent characteristics of Tuina,such as its individualized application and reliance on somatic sensation,pose significant challenges to the successful implementation and assessment of blinding in clinical trials,directly compromising the reliability of findings and the hierarchy of evidence. This article aims to provide a systematic review of the status,core difficulties,and methodological advancements in blinding within Tuina clinical trials. It thoroughly analyzes the pivotal role of blinding in controlling measurement and performance biases,while highlighting the severe realities within the field,including low rates of blinding reporting,insufficient scientific design of sham interventions,and simplistic,non-standardized assessment methods. To address these challenges,the article introduces the DITTO framework,suitable for procedural interventions,offering a structured approach for the systematic design of sham Tuina protocols. Furthermore,it discusses the value and limitations of quantitative tools like the James Blinding Index(JBI)and Bang Blinding Index(BBI)for assessing blinding quality,advocating for the establishment of a multi-role,multi-dimensional evaluation system. Finally,the article foresees future directions,such as integrating modern technologies like Virtual Reality(VR)/Augmented Reality(AR)to optimize blinding design, conducting large-scale methodological studies to establish Tuina-specific standards,and strengthening academic transparency. This review concludes that only through methodological innovation and standardization can the scientific rigor of Tuina clinical research be fundamentally enhanced,thereby generating compelling high-level evidence for its efficacy.
