针刺联合补阳还五汤加减治疗气虚血瘀型慢性心力衰竭的疗效及对患者心功能的影响

摘要:

[目的] 探究针刺联合补阳还五汤加减治疗气虚血瘀型慢性心力衰竭的疗效及对患者心功能的影响。[方法] 选取140例于2022年1月—2023年6月在淄博市中医医院进行治疗的气虚血瘀型慢性心力衰竭患者，按随机数字表法将其分为对照组70例，观察组70例。对照组给予补阳还五汤加减，观察组给予岐黄针联合补阳还五汤加减治疗。评估中医证候积分水平；检测心排血量（CO）、左心室射血分数（LVEF）、B型利钠肽（BNP）、血管紧张素Ⅱ（AngⅡ）、血醛固酮（ALD）、肾素活性（PRA）、炎症反应指标水平，对比两组临床治疗效果。[结果] 治疗后，与对照组比较，观察组气短、乏力、倦怠懒言、自汗积分水平、BNP、AngⅡ、ALD、PRA、肿瘤坏死因子-α（TNF-α）、超敏C-反应蛋白（hs-CRP）水平降低（P＜0.05），CO、LVEF水平升高（P＜0.05）；总有效率观察组优于对照组（P＜0.05）。[结论] 经岐黄针联合补阳还五汤加减进行治疗，能降低中医证候积分情况，抑制氧化应激状态，改善血管内皮功能，恢复心功能，治疗效果较为显著。

Abstract:

[Objective] To explore the curative effect of acupuncture combined with Buyang Huanwu Decoction in treating chronic heart failure with qi deficiency and blood stasis and its influence on patients’ heart function. [Methods] A total of 140 patients with chronic heart failure with qi deficiency and blood stasis in Zibo Hospital of Traditional Chinese Medicine from January 2022 to June 2023 were divided into 70 cases in control group and 70 cases in observation group by random number method. The control group was treated by modified Buyang Huanwu Decoction and the observation group was treated by Qihuang needle combined with modified Buyang Huanwu Decoction. To evaluate the integral level of traditional Chinese medicine syndrome；test cardiac output （CO），left ventricular ejection fraction （LVEF），type B natriuretic peptide （BNP），angiotensin （Ang），blood aldosterone （ALD），renin activity （PRA），and compare the clinical treatment effect between the two groups. [Results] After treatment，compared with the control group，the levels of shortness of breath，fatigue，lethargy，spontaneous sweating score，BNP，AngⅡ，ALD，PRA，TNF-α and hs-CRP in the observation group were decreased （P＜0.05），and the levels of CO and LVEF were increased （P＜0.05）. The total effective rate of the observation group was better than the control group（P＜0.05）. [Conclusion] Qihuang needle combined with Buyang Huanwu Decoction can reduce traditional Chinese medicine syndrome score，inhibit oxidative stress，improve vascular endothelial function and restore heart function，and the therapeutic effect is significant.
