姜桂益瘿汤治疗脾肾阳虚型桥本氏甲状腺功能减退症的临床观察

摘要:

[目的] 观察姜桂益瘿汤联合小剂量左甲状腺素钠治疗脾肾阳虚型桥本氏甲状腺炎合并甲状腺功能减退症患者的疗效。[方法] 将 60例患者随机分为对照组(西药)和治疗组(西药+中药),各30例,两组均以4周为1个疗程,共治疗3个疗程。[结果] 两种方法均可升高血清总三碘甲状腺原氨酸(T₃)、血清总甲状腺素(T₄)并降低血清促甲状腺素(TSH)水平,治疗前后各组内比较差异有统计学意义(P<0.05);治疗后治疗组TSH明显低于对照组,T₃、T₄高于对照组,差异有统计学意义(P<0.05).在中医证候总疗效、中医证候积分,面肢水肿、神疲乏力、腰膝酸软、畏寒肢冷、纳呆腹胀症状,治疗组明显好于对照组,差异有统计学意义(P<0.05).[结论] 姜桂益瘿汤治疗脾肾阳虚型桥本氏甲状腺功能减退症有较好疗效,中西医结合疗效优于纯西药治疗。

Abstract:

[Objective] Observing the affect of Jianggui Yiying prescription with small dose of levothyroxine on treating the spleen kidney Yang deficiency type of hashimoto's thyroiditis with hypothyroidism. [Methods] Sixty cases were randomly divided into control group (western medicine) and treatment group (western medicine and traditional Chinese medicine), 30 cases in each group, 4 weeks as 1 course of treatment, treatment for 3 periods. [Results] Two methods all can increase T₃, T₄ and decrease TSH, before and after treatment the difference intra-group was statistically significant (P<0.05), TSH in the treatment group significantly lower than the control group after treatment, T₃, T₄ was higher than the control group, the difference was statistically significant(P<0.05). In TCM syndrome total efficacy, TCM syndrome integral, floating face limb swelling, god fatigue, cold, lumbar debility, and cold limbs for symptoms of abdominal distension, contrast treatment group was obviously better than the control group, the difference was statistically significant(P<0.05). [Conclusion] Jianggui Yiying decoction on spleen and kidney Yang deficiency type hashimoto's thyroiditis has good curative effect, curative effect of combination traditional Chinese and western medicine is superior to pure western medicine treatment.
