Clinical study on acupuncture “Xingnao Kaiqiao” for prevention of stroke-associated pneumonia

Abstract
    [Objective] To observe the preventive effect of the “Xingnao Kaiqiao” acupuncture technique on stroke-associated pneumonia(SAP) and to preliminarily explore its potential mechanism from the perspective of immune inflammation. [Methods] Patients with acute cerebral infarction within 24 hours of onset were enrolled from the First Affiliated Hospital of Tianjin University of Chinese Medicine and randomly assigned to an experimental group or a control group. The control group received conventional Western medical treatment,while the experimental group received additional “Xingnao Kaiqiao” acupuncture for 30 minutes once daily for 7 days. The incidence of SAP,National Institutes of Health Stroke Scale(NIHSS) scores,Glasgow Coma Scale(GCS) scores,and Functional Oral Intake Scale(FOIS) scores were observed in both groups. Levels of peripheral white blood cell count(WBC),neutrophil percentage(NEUT%),absolute neutrophil count(NEUT),absolute lymphocyte count(LYMPH),C-reactive protein(CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-4(IL-4),and interferon-γ(IFN-γ) were measured. [Results] Sixty-three patients were finally included(control group:n=32;experimental group:n=31). Compared with the control group,the experimental group showed a significantly lower incidence of SAP(P<0.05). Compared with pre-treatment,post-treatment NIHSS scores decreased significantly(P<0.01),while GCS and FOIS scores increased significantly(P<0.01) in both groups,with greater improvement observed in the experimental group(P<0.01). Compared with pre-treatment,WBC,NEUT%,NEUT and CRP levels decreased significantly(P<0.01) in both groups after treatment,with a greater reduction in the experimental group(P<0.05). LYMPH levels increased significantly(P<0.01),with a superior increase in the experimental group(P<0.05). Compared with pre-treatment,TNF-α,IL-4 and IL-6 levels decreased significantly(P<0.01),while IFN-γ levels increased significantly(P<0.01) post-treatment in both groups,and the improvement in these cytokine indicators was more pronounced in the experimental group(P<0.01). [Conclusion] The “Xingnao Kaiqiao” acupuncture technique can effectively prevent SAP,improve consciousness,promote neurological recovery,and alleviate swallowing dysfunction. Its mechanism may be associated with the regulation of the immune-inflammatory response.
