Clinical study on modified Gegen Qinlian Decoction combined with mFOLFOX6 neoadjuvant chemotherapy for patients with low rectal cancer

Abstract
    [Objective] To study the effects and safety of modified Gegen Qinlian Decoction in patients with low rectal cancer underwent mFOLFOX6 neoadjuvant chemotherapy and sequential laparoscopic surgery,to observe and analyze the incidence of neoadjuvant chemotherapy side effects,operative efficiency and post-operative complications. [Methods] Patients with resectable locally advanced low rectal cancer admitted to Tianjin Fourth Central Hospital from October 2021 to February 2023 were randomly divided into treatment group and control group. There were 26 patients in the control group,which underwent mFOLFOX6 neoadjuvant chemotherapy and laparoscopic surgery. The treatment group was given modified Gegen Qinlian Decoction on the basis of the control group. The side effects of neoadjuvant chemotherapy,surgical efficiency,immune function and post-operative complications were compared between these two groups. [Results] 1)The incidence of neoadjuvant chemotherapy side effects such as myelosuppression,nausea-vomiting,diarrhea and peripheral neurotoxicity in treatment group were significantly less than that of the control group(P<0.05). The level of CD3+,CD4+/CD8+ 7 days after chemotherapy in treatment group were significantly higher than that of the control group(P<0.01). The level of CD4+,CD8+ 7 days after chemotherapy in treatment group were significantly less than that of the control group(P<0.01). 2)For surgical efficiency and quality evaluation,operative time,volume of intraoperative hemorrhage,days of hospitalization,incidence of urinary retention were significantly less than that of control group. And first post-operative anal ventilation time was significantly less than that of control group (P<0.05 or P<0.01). 3)All of them were statistically different. For post-operative immune function evaluation,the level of CD3+,CD4+ and CD4+/CD8+ 7 days after surgery in treatment group were statistically higher than that of control group. And the level of CD8+ in treatment group was statistically less than that of control group (P<0.01). [Conclusion] For patients with low rectal cancer undergoing mFOLFOX6 neoadjuvant chemotherapy and sequential laparoscopic surgery,adding Gegen Qinlian Decoction can reduce the toxic and side effects of chemotherapy drugs on the damage of immune function,which may help to create favorable conditions for surgery,shorten the operative time,reduce the amount of blood loss,promote the rapid recovery of gastrointestinal function after surgery,and reduce the incidence of urinary retention.
