| Abstract
|
| [Objective] To comprehensively assess the clinical efficacy and safety of combining blood purification and Xuebijing Injection therapy for sepsis through Meta-analysis,thereby providing evidence-based support for clinical practice. [Methods] We systematically searched CNKI,CBM,VIP,Wanfang,PubMed,EMbase,Cochrane Library and Web of Science databases, screened randomized controlled trials(RCTs) that studied this treatment as of July 20,2025. The quality of studies and potential bias were assessed by RevMan 5.3 to perform the Meta-analysis. [Results] Twenty-two RCTs involving 1 860 patients were included. The Meta-analysis demonstrated superior overall response rates in the combined therapy group versus controls [OR=4.03,95%CI(2.68,6.06),P<0.001]. The combined treatment reduced inflammatory markers including WBC count [WMD=-5.53,95%CI(-7.86,-3.20),P<0.001],CRP [WMD=-15.09, 95%CI(-21.53,-8.65),P<0.001],PCT [SMD=-1.64,95%CI(-2.13,-1.15),P<0.001],TNF-α [SMD=-2.47,95%CI(-3.48,-1.47),P<0.001],and IL-6 [WMD=-46.44,95%CI(-67.61,-25.27),P<0.001]. Improved hemodynamic and renal function included cardiac index [SMD=0.93,95%CI(0.64,1.24),P<0.001],MAP [WMD=5.40,95%CI(4.24,6.57),P<0.001],BUN [SMD=-2.27,95%CI(-3.33,-1.20),P<0.001],and Scr [SMD=-2.76,95%CI(-3.85,-1.66),P<0.001],Reduced APACHEⅡ scores [WMD=-3.46,95%CI(-4.02, -2.90),P<0.001] and SOFA scores [WMD=-2.02,95%CI(-3.05,-0.99),Reduced mortality[OR=0.41,95%CI(0.27,0.62),P<0.001],with favorable safety [OR=0.44,95%CI(0.10,1.99),P=0.29]. [Conclusion] Blood purification combined with Xuebijing Injection,as an integrated Chinese-Western medicine strategy, shows significant clinical efficacy in sepsis patients. It effectively reduces systemic inflammatory factor release, improves hemodynamics and renal function, leading to improvements in APACHE Ⅱ and SOFA scores, reduced mortality, and demonstrates good safety. |
