摩腹法对肠易激综合征白兔模型不同脑区激活特征的影响

摘要:
[目的]探讨摩腹法对便秘型肠易激综合征(IBS)白兔模型不同脑区激活的功能磁共振(fMRI)特征。[方法]将新西兰白兔随机分为正常组、模型组和摩腹组,采用冰水灌胃与冰敷束缚相结合制备便秘型IBS动物模型。经摩腹干预,在第1次摩腹后即刻和第20次后两个时点观察3组动物功能磁共振脑区激活变化。[结果]正常组白兔脑区激活部位在丘脑、扣带前回、脑岛皮质。模型组fMRI脑区激活部位在丘脑、扣带前回、脑岛皮质、脑干和小脑,其中在丘脑和扣带前回激活区域,模型组的激活象素和强度明显高于正常组(P<0.05)。经摩腹第1次操作后,激活部位在丘脑、扣带前回、脑岛皮质、脑干;操作20次后,激活部位在丘脑、扣带前回、脑岛皮质,摩腹组激活脑区与模型组比较激活像素和激活强度显著降低(P<0.01)。[结论]摩腹法可以有效调控便秘型肠易激综合征家兔模型内脏敏感化中枢。

Influence of rubbing abdomen on regional encephalic activation in rabbits with irritable bowel syndrome

Abstract:
[Objective] To investigate the fMRI characteristic and clinical effects of Rubbing abdomen on regional encephalic activation in rabbits with constipation-predominant irritable bowel syndrome (C-IBS). [Methods] New Zealand male albino rabbits were randomly divided into normal group, model group and Rubbing abdomen group. C-IBS was induced in all rabbits by the combined ice water intragastric administration and ice compress constraint. Encephalic regions of fMRI were observed before and after the first massage and 20 times treatment later. [Results] The activated regions were thalamencephal, Anterior Cingulate Cortex (ACC) and insula cortex (IC) in normal group, and were thalamencephal, ACC, insula cortex, brain stem, cerebellum in model group. Activated regions were thalamencephal, ACC, IC and brain stem after the first massage in Rubbing abdomen group, however the activated regions were the former three areas after 20 times. The activated encephalic intensity and range in Rubbing abdomen group were significantly lower than that in model group (P<0.01). [Conclusion] Rubbing abdomen can effectively regulate sensitization center of viscera in rabbits with C-IBS.
