超微针刀治疗腰背肌筋膜炎的临床疗效观察

摘要:

[目的] 观察超微针刀疗法治疗腰背肌筋膜炎的临床疗效。[方法] 研究共采集患者90例，随机分为治疗组和对照组，治疗组取腰背部肌筋膜触发点以超微针刀疗法治疗，对照组采用阿是穴和膀胱经、华佗夹脊穴循经穴位毫针针刺治疗。以上治疗均为隔日治疗1次，7 d为1个疗程，共治疗2个疗程。采用简化麦吉尔（Mcgill）疼痛问卷对两组患者治疗前后分别进行疼痛评定；同时采集治疗前后腰背肌核心肌群肌电信号，比较治疗前后的平均肌电波幅（AEMG）和平均功率频率（MPF），并对两组治疗后的临床疗效进行比较。[结果] 治疗组的总有效率（88.10%）明显高于对照组（80.49%）（P<0.05）。治疗后，两组患者的疼痛分级指数评分（PRI）、疼痛视觉模拟评分（VAS）、现时疼痛强度评分（PPI）均较治疗前显著降低（P<0.05），且观察组治疗后的PRI、VAS、PPI评分显著低于对照组（P<0.05）。治疗后腰背肌核心肌肉的AEMG、MPF均增加，差异有统计学意义（P<0.05）。[结论] 超微针刀疗法对于腰背肌筋膜炎疗效确切，有临床推广应用的价值。

Abstract:

[Objective] To observe the clinical effect of the ultrastructural needle knife therapy in the treatment of lumbar and dorsal myofascitis.[Methods] A total of 90 patients were randomly divided into treatment group and control group. The control group was treated with filiform acupuncture at Ashi point,bladder meridian and HuatuoJiaji point. The above treatments were treated once every other day,7 days as a course of treatment,a total of 2 courses of treatment. The scores of simplified McGill pain questionnaire were used to evaluate the pain of the two groups before and after treatment,and the EMG signals of the core muscles of low back muscle were collected before and after treatment,the average EMG amplitude (AEMG) and average power frequency (MPF),and the clinical effects of the two groups after treatment were compared.[Results] The total effective rate of the treatment group (88.10%) was significantly higher than that of the control group (80.49%) (P<0.05). After treatment,the scores of PRI,VAS and PPI in the two groups weresignificantly lower than those before treatment (P<0.05),and the scores of PRI,VAS and PPI in the observation group were significantly lower than those in the control group(P<0.05).[Conclusion] The ultramicroneedle knife therapy is effective in the treatment of lumbar dorsal myofascitis and has the value of clinical application.
