益肾暖宫汤联合针灸治疗多次体外受精-胚胎移植患者疗效及对妊娠结局、性激素水平的影响

摘要:

[目的] 探讨益肾暖宫汤联合针灸治疗多次体外受精-胚胎移植（IVF-ET）患者疗效及对妊娠结局、性激素水平的影响。[方法] 将无锡市人民医院生殖中心2017年7月—2018年7月收治的150例符合试验病例标准的IVF-ET患者采用随机数字表法分为观察组和对照组，每组75例。对照组给予试管黄体期长方案联合针灸治疗，观察组予以益肾暖宫汤加减联合针灸治疗，观察其对临床疗效、妊娠率和性激素水平的影响。[结果] 两组取卵数、受精率和受精卵分裂率比较差异无统计学意义（P>0.05），观察组优质卵率、优质胚胎率显著高于对照组（P<0.05）；观察组生化妊娠率和临床妊娠率均显著高于对照组（P<0.05）；观察组卵泡生成激素（FSH）、黄体生成激素（LH）、雌二醇（E2）、孕酮（P）水平显著高于对照组（P<0.05）；治疗前两组中医证状积分比较无统计学差异（P>0.05），治疗后两组中医证候积分均有显著下降（P<0.05），且观察组显著低于对照组（P<0.05）；观察中医证候疗效（88.00%）显著高于对照组（69.33%），差异有统计学意义（P<0.05）。[结论] 益肾暖宫汤联合针灸治疗多次IVF-ET有良好疗效，但是否由两者共同作用所致，仍需进一步研究证实。

Abstract:

[Objective] To investigate the therapeutic effect of Yishen Nuangong Decoction combined with acupuncture on multiple in vitro fertilization and embryo transfer (IVF-ET) patients and its influence on pregnancy outcome and sex hormone level.[Methods] The 150 patients with IVF-ET who met the criteria of experimental cases were divided into observation group and control group by random number table method,75 cases in each group. The control group was treated with long-term routine combined with acupuncture and moxibustion treatment of corpus luteum in vitro,while the observation group was treated with modified Yishen Nuangong Decoction combined with acupuncture and moxibustion to observe its effect on clinical efficacy,pregnancy rate and sex hormone level.[Results] There was no significant difference in the number of ova taken,fertilization rate and cleavage rate of fertilized ova between two groups (P>0.05). The rate of high-quality ova and high-quality embryos in the observation group were significantly higher than those in the control group (P<0.05),and the biochemical pregnancy rate and clinical pregnancy rate in the observation group were significantly higher than those in the control group (P<0.05). The levels of follicle-forming hormone (FSH),luteinizing hormone (LH),estradiol (E2) and progesterone (P) in the observation group were significantly higher than those in the control group (P<0.05). There was no significant difference in TCM syndrome scores between two groups before treatment (P>0.05). After treatment,the traditional Chinese medicine syndrome scores of the two groups decreased significantly (P<0.05),and the observation group was significantly lower than the control group (P<0.05). The curative effect of TCM syndrome (88.00%) was significantly higher than that of the control group (69.33%) with significant difference (P<0.05).[Conclusion] Yishen Nuangong decoction combined with acupuncture has a good effect in treating multiple IVF-ET,whether it is caused by the combined effect needs further research to confirm.
