清胰汤联合乌司他丁治疗重症急性胰腺炎的疗效及其对患者免疫功能的影响

摘要:

[目的] 探究清胰汤联合乌司他丁治疗重症急性胰腺炎的疗效及其对患者免疫功能的影响。[方法] 选取2020年9月—2021年8月100例苏州市中西医结合医院重症急性胰腺炎患者为研究对象，采用随机数字表法随机分为对照组及治疗组各50例。对照组使用乌司他丁进行注射治疗，治疗组在对照组的基础上联合清胰汤进行治疗。对比两组病情恢复情况，检测两组中医证候评分，酶联免疫吸附法检测肠道黏膜屏障功能、胰腺功能、白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）、白细胞介素-8（IL-8）水平，免疫透射比浊法检测免疫球蛋白（Ig）G、IgA、IgM水平，对比两组治疗效果。[结果] 治疗后，与对照组比较，治疗组中医证候评分、D-乳酸（D-lactose）、二胺氧化酶（DAO）、胰高血糖素（GLU）、淀粉酶（AMY）、胰蛋白酶原-2（TPS-2）、IL-6、TNF-α、IL-8水平降低，差异均具有统计学意义（P<0.05）；胰岛素（INS）、IgG、IgA、IgM升高，差异均具有统计学意义（P<0.05）；治疗组临床治疗有效率高于对照组，差异具有统计学意义（P<0.05）。[结论] 使用清胰汤联合乌司他丁对重症急性胰腺炎患者进行治疗，中医证候评分下降，肠道黏膜屏障功能、胰腺功能、免疫功能均得到有效改善，炎症反应减轻，整体治疗效果显著。

Abstract:

[Objective] To explore the curative effect of Qingyi Decoction combined with ulinastatin in the treatment of severe acute pancreatitis and its influence on the immune function of patients.[Methods] A total of 100 patients receiving severe acute pancreatitis treatment in our hospital from September 2020 to August 2021 were selected,the 100 patients were divided into control group and treatment group with 50 cases in each group by random number table method. The control group was treated with ulinastatin injection,and the treatment group was treated with Qingyi Decoction on the basis of the control group. The recovery of the two groups was compared,the score of traditional Chinese medicine(TCM) syndrome was detected,and the intestinal mucosal barrier function,pancreatic function,interleukin-6 (IL-6) and tumor necrosis factor-a (TNF-α) was detect by enzyme-link immunosorbent assay Interleukin-8 (IL-8);the levels of immunoglobulin G (IgG),immunoglobulin A (IGA) and immunoglobulin M (IgM) was detect by immunoturbidimetry.[Results] After treatment,compared with the control group,the TCM syndrome score,D-lactate,diamine oxidase (DAO),glucagon (GLU),amylase (AMY),trypsinogen-2 (TPS-2),IL-6 and TNF-α in the treatment group was significantly higher,the levels of IL-8 is significantly lower (P<0.05);the levels of insulin (INS),IgG,IgA and IgM was significantly increase (P<0.05);the clinical effective rate of the treatment group was higher than that of the control group,with statistical difference (P<0.05).[Conclusion] Using Qingyi Decoction combined with ulinastatin in the treatment of severe acute pancreatitis,TCM syndrome score decreased,intestinal mucosal barrier function,pancreatic function and immune function were effectively improved,inflammatory response was reduced,and the overall treatment effect was significant.
