益气活血利水汤配合脏时相调法针刺治疗宫颈癌术后尿潴留的临床疗效分析

摘要:

[目的] 探讨益气活血利水汤配合脏时相调法针刺治疗宫颈癌术后尿潴留的临床疗效。[方法] 2021年12月—2022年12月纳入112例宫颈癌术后尿潴留患者，按照随机数字表法将患者分为两组，每组各56例。两组均给予留置导尿、物理刺激训练膀胱功能，在此基础上，对照组采用益气活血利水汤治疗，观察组采用益气活血利水汤联合脏时相调法针刺治疗。比较两组疗效、中医证候积分、恢复自主排尿时间、平均每次排尿时间、尿动力学、膀胱过度活动综合征评分（OABSS）和不良反应差异。[结果] 观察组治疗总有效率高于对照组（96.343% vs. 80.36%，P＜0.05），恢复自主排尿时间、平均每次排尿时间均短于对照组（P＜0.05）。治疗后两组小便短少、面色淡白、疲倦无力、四肢不温、心慌失眠、胸胁疼痛证候积分以及膀胱最大容量、最大尿流率、逼尿肌压力、残余尿量、OABSS评分较治疗前降低（P＜0.05），且观察组治疗后上述中医证候积分以及膀胱最大容量、最大尿流率、逼尿肌压力、残余尿量、OABSS评分低于对照组（P＜0.05）。[结论] 益气活血利水汤配合脏时相调法针刺可显著改善膀胱功能，提高临床疗效且安全性高。

Abstract:

[Objective] To investigate the clinical effect of Yiqi Huoxue Lishui Decoction combined with zangshi phase regulation acupuncture on urinary retention after cervical cancer surgery. [Methods] From December 2021 to December 2022，112 patients with urinary retention after cervical cancer surgery were selected and divided into two groups with 56 cases in each group by digital table method. Both groups were given indwelling catheterization and physical stimulation to train bladder function. On this basis，the control group was treated with Yiqi Huoxue Lishui Decoction，and the observation group was treated with Yiqi Huoxue Lishui Decoction combined with zangshi phase regulation acupuncture. The efficacy，traditional Chinese medicine（TCM） symptom score，recovery time of spontaneous urination，mean time per urination，urodynamics，overactive bladder syndrome score（OABSS） and adverse reactions were compared between the two groups. [Results] The total effective rate of the observation group was higher than that of the control group（96.343% vs. 80.36%，P＜0.05），and the recovery time of spontaneous urination and the average time of each urination were shorter than that of the control group（P＜0.05）. After treatment，the symptoms of shortness of urination，pale face，fatigue and weakness，warm limbs，palpidity and insomnia，thoracic and hypocholic pain，maximum bladder volume，maximum urine flow rate，detrusor pressure，residual urine volume and OABSS score in the two groups were decreased compared with those before treatment（P＜0.05）. After treatment，the TCM syndrome score，maximum bladder volume，maximum urine flow rate，detrusor pressure，residual urine volume and OABSS score in the observation group were lower than those in the control group（P＜0.05）. [Conclusion] Yiqi Huoxue Lishui Decoction combined with zangshi phase regulation acupuncture can significantly improve bladder function；improve clinical efficacy and high safety.
