| 糖尿病肾脏疾病(DKD)已经成为全世界范围内公共健康问题,作为终末期肾脏的主要病因DKD的防治十分关键。足细胞损伤是糖尿病肾病形成与发展的重要病理基础,主要损伤形态包括足细胞肥大、足细胞上皮间质转化、足细胞脱离和足细胞凋亡,并且与自噬、炎症反应及氧化应激、上皮间质转化(EMT)、线粒体功能障碍等多个方面有关。研究表明转化生长因子1(TGF-β1)/Smad信号通路在足细胞的损伤中起到了重要作用,同时随着中医药的不断发展,其多靶点、多组分、多环节、低不良反应的优势逐渐被认可,在临床使用中与西药搭配得当,可收到事半功倍的疗效。近年来研究表明中药单体及复方能够有效减轻足细胞损伤,进而改善肾功能。基于此,文章查阅相关文献,综述了TGF-β1/Smad信号通路的基本结构及其与DKD的关系,中医药调控该信号通路改善足细胞损伤保护肾脏功能的研究现状,旨在为DKD的防治提供新思路和方向。 |

| Diabetic kidney disease(DKD) has become a worldwide public health problem and the prevention and treatment of DKD as the main cause of end-stage renal is critical. Podocyte injury is an important pathological basis for the formation and progression of diabetic nephropathy,and the main injury patterns include podocyte hypertrophy,podocyte epithelial-mesenchymal transition,podocyte detachment,and podocyte apoptosis,and are associated with autophagy,inflammatory response,and oxidative stress,epithelial-mesenchymal transition(EMT),mitochondrial dysfunction,and many other aspects. Studies have shown that the transforming growth factor 1(TGF-β1)/Smad signaling pathway plays an important role in foot cell injury,while with the continuous development of traditional Chinese medicine(TCM),its multi-target,multi-component,multi-faceted,low adverse reaction advantages are gradually recognized,in clinical use and western medicine with proper matching,can receive twice the effect with half the effort. In recent years,studies have shown that traditional Chinese medicine monomers and compound formulas can effectively reduce podocyte injury and thus improve renal function. The review summarizes the basic structure of the TGF-β1/Smad signaling pathway and its relationship with DKD. It also explores the current research status of TCM in regulating this signaling pathway to improve podocyte damage and protect renal function. The aim is to provide new ideas and directions for the prevention and treatment of DKD. |
