火龙罐综合灸治疗产后抑郁合并心脾两虚型睡眠障碍的效果分析

摘要:

目的 研究分析火龙罐综合灸对产后抑郁伴心脾两虚型睡眠障碍患者的临床效果。方法 选取医院2023年9月—2024年10月诊治的130例产后抑郁伴心脾两虚型睡眠障碍患者进行研究，按随机数字表法分为观察组和对照组，每组65例。对照组患者给予常规护理干预，观察组患者给予火龙罐综合灸治疗。比较两组患者的中医证候积分、负性情绪[焦虑自评量表（SAS）及抑郁自评量表（SDS）]、睡眠质量[匹兹堡睡眠质量指数量表（PSOI）]、生活质量[生活质量综合评定问卷（GQOLI-74）]的变化情况和差异情况。结果 不同时间点两组患者的中医证候积分、负性情绪SAS、SDS评分均存在时间效应、组间效应及交互效应（P＜0.05）；干预2周时观察组患者的负性情绪SAS、SDS评分、中医证候积分均低于对照组（P＜0.05）；干预4周后观察组患者的负性情绪SAS、SDS评分、中医证候积分均低于对照组（P＜0.05）；干预4周后观察组患者的睡眠质量改善情况优于对照组（P＜0.05）；干预4周后观察组患者的生活质量提升情况优于对照组（P＜0.05）。结论 对于产后抑郁合并心脾两虚型睡眠障碍患者，采用火龙罐综合灸治疗，可有效改善患者的睡眠障碍情况，提升睡眠质量，且患者的负性情绪得到有效缓解，生活质量提升。

Abstract:

Objective To study and analyze the clinical effect of fire dragon pot moxibustion on postpartum depression and sleep disorder with deficiency of heart and spleen.Methods A total of 130 patients with postpartum depression and sleep disorder with deficiency of heart and spleen in our hospital from September 2023 to October 2024 were selected for the study. They were divided into observation group and control group according to random number table method, with 65 cases in both. Control group patients were given routine nursing intervention, observation group patients were given dragon pot comprehensive moxibustion treatment. The changes and differences of traditional Chinese medicine(TCM) syndrome scores, negative emotions [self-rating anxiety scale(SAS) and self-rating depression scale(SDS)], sleep quality [Pittsburgh sleep quality index(PSOI)] and quality of life(GQOLI-74) were compared between the two groups.Results The results of repeated measurement ANOVA showed that the TCM syndrome scores, SAS and SDS scores of patients in the two groups at different time points had temporal effects, inter-group effects and interaction effects(P < 0.05). After 2 weeks of intervention, compared with the control group, the TCM syndrome scores, SAS and SDS scores of the observation group were lower(P < 0.05). After 4 weeks of intervention, compared with the control group, the TCM syndrome scores, negative emotion SAS and SDS scores of the observation group were lower(P < 0.05). After 4 weeks of intervention, compared with the control group, the sleep quality of observation group was better(P < 0.05). After 4 weeks of intervention, compared with the control group, the quality of life in the observation group was better(P < 0.05).Conclusion For patients with postpartum depression combined with heart and spleen deficiency sleep disorders, the use of dragon pot comprehensive moxibustion treatment can effectively improve the patients' sleep disorders, improve sleep quality, and the patients' negative emotions can be effectively alleviated, and the quality of life can be improved.
