从“黜浊培本”论治癌因性疲乏

摘要:

癌因性疲乏（CRF）是肿瘤最常发生的症状之一，严重影响患者的身心健康状况。不同于普通的疲乏，CRF的患者经休息或睡眠无法有效缓解机体的不适，CRF的发生由肿瘤引起，并伴随着肿瘤的发生发展而变化。发病率高且病情隐匿，发病快且难以缓解，病势严重，病程迁延不愈是癌乏的主要特点。目前，CRF的发生机制尚未阐明。西医治疗手段有限，中医药在治疗方面具有独特优势。基于“黜浊培本”理论的“扶正解毒祛瘀法”治疗CRF，取得了良好疗效。癌乏的发生在于“虚”“毒”“瘀”三者之间相互转化，相互影响，贯穿癌症进展全过程。CRF多由正气亏虚，邪毒蕴结所致，治疗上以扶正为纲，化瘀为常，兼顾解毒，临床上已取得明显效果，文章拟对癌因性疲乏的病因病机及治疗原则展开探讨，并通过医案进行分析，以期为癌乏的治疗提供借鉴的思路。

Abstract:

Cancer-realted fatigue(CRF) is one of the most common symptoms of cancer, which seriously affects the physical and mental health of patients. Different from ordinary fatigue, patients with CRF cannot effectively relieve the discomfort of the body through rest or sleep. The occurrence of CRF is caused by tumors and changes with the development of tumors. High incidence and hidden disease, rapid onset and difficult to relieve, serious disease and prolonged course of disease are the main characteristics of cancer. At present, the mechanism of CRF has not been clarified. Western medicine has limited means of treatment, while traditional Chinese medicine has unique advantages in treatment. Based on the theory of "Chuzhuo Peiben", the method of "Fuzheng Jiedu Quyu method" to treat CRF, which has achieved good curative effect. The occurrence of cancer lies in the mutual transformation and mutual influence between the "vacuity" "poison" and "stasis", which runs through the whole process of cancer progression. CRF is mostly caused by the deficiency of healthy qi and the accumulation of evil poison. In the treatment, the main line is to support integrity, the removal of blood stasis is often considered, and detoxification has been achieved in clinical practice. This paper intends to discuss the etiology, pathogenesis and treatment principle of cancer-related fatigue, and analyze medical records to provide reference ideas for the treatment of cancer fatigue.
