| 国医大师翁维良治疗儿童高度房室传导阻滞经验 |

| 房室传导阻滞是缓慢性心律失常的1种类型，其中高度房室传导阻滞是指阻滞部位位于希氏束或希氏束以下，可能会进展为三度房室传导阻滞，预后较差，儿童房室传导阻滞临床中较为罕见，多无临床症状且病因常常不明，为治疗增加了难度，目前儿童房室传导阻滞患者家长对西医治疗接纳度较低，中医药治疗以其用药灵活，安全性高的特点发挥巨大的作用。翁维良教授认为治疗儿童房室传导阻滞时，由于小儿“脏气清灵，稚阴稚阳”的体质特点，用药与成人有所不同，在温阳活血的基本治疗原则下，用药更为轻灵平和，在心脏传导功能有了明显恢复后，即减少活血药的使用，而以益气、养阴、温阳之法促进其心脏功能的自主恢复，文章报道翁维良教授治疗儿童高度房室传导阻滞病案，以期为临床诊疗提供启发。 |

| Atrioventricular block（AVB） is a type of bradyarrhythmia，high-grade AVB，characterized by block at or below the His bundle，may progress to third-degree AVB and is associated with a poor prognosis. AVB is relatively rare in children，often asymptomatic and of unknown etiology，which complicates clinical management. Currently，parents of pediatric AVB patients exhibit low acceptance of conventional Western treatments. Traditional Chinese medicine（TCM），with its flexible therapeutic strategies and high safety profile，plays a significant role in such cases. Professor WENG Weiliang emphasizes that pediatric AVB treatment should account for the unique constitutional characteristics of children-described in TCM as “delicate visceral qi，immature yin and yang”. Unlike adults，children require gentler and more balanced pharmacological interventions. Under the fundamental principle of warming yang and activating blood circulation，Professor WENG Weiliang tailors the treatment to the child’s physiological needs. Once cardiac conduction shows significant improvement，he reduces the use of blood-activating herbs and shifts the focus to replenishing qi，nourishing yin，and warming yang to promote autonomous recovery of cardiac function. This paper presents the first documented case of high-grade AVB in a child successfully treated by Professor WENG Weiliang，aiming to provide insights for the clinical management of this condition. |
