善胃Ⅰ号方对血瘀热毒型胃癌前病变PCNA的调节*

摘要:

[目的]观察中药善胃Ⅰ号方对血瘀热毒型胃癌前病变的治疗效果及增殖细胞核抗原(PCNA)治疗后的表达情况。[方法]将内镜和病理确诊为慢性萎缩性胃炎伴胃黏膜异型增生且符合中医血瘀热毒证型的82例患者,随机分为治疗组和对照组。治疗组34例,给予善胃Ⅰ号方;对照组48例,给予猴头菌片。两组均用药6个月。[结果]1)活血解毒之善胃Ⅰ号方治疗后总有效率为85.29%,优于对照组29.17%(P<0.01)。2)病理改善:治疗后萎缩、肠化、不典型增生均有明显改善,明显优于对照组(P<0.01);3)治疗后黏膜PCNA表达显著降低,明显优于对照组(P<0.05)。[结论]善胃Ⅰ号方对血瘀热毒型胃癌前病变有良好的治疗作用,并可使异型增生程度逆转,降低PCNA的表达,对胃癌有一定的预防作用。

Abstract:

[Objective] To observe curative effect of TCM Shanwei first formula on regualting PCNA expression of blood stagnation and pyretic toxicity syndrome of stomach precancerosis patients.[Methods] The selected eighty-two patients were randomly divided into treatment group and control group.The patients of treatment group were adminstrated Shanwei first formula and that of treatment group were adminstrated heriacium tablet.The course of treatment was six months.[Results] The total effective rate of treatment group was 85.29%and that of control group was 29.17% (P<0.01).The improvement extent of atrophia,Intestinal metaplasia,atypical hyperplasia in treatment group was obviously higher than that in control group (P<0.01). The lower PCNA expression of treatment group was obviously better than that of control group (P<0.05).[Conclusions] Shanwei first formula may have a better curative and preventive effect of blood stagnation and pyretic toxicity syndrome of stomach precancerosis patients and may deteriorate atypical hyperplasia and decrease PCNA expression.
