芪明颗粒对早期糖尿病肾病炎症因子及尿微量白蛋白的影响

摘要:

[目的] 通过观察芪明颗粒对早期糖尿病肾病炎症因子及尿微量白蛋白的影响，探索其治疗早期糖尿病肾病作用机制。[方法] 选入符合纳入标准的早期糖尿病肾病患者120例，采用简单数字表的随机方法分为治疗组60例和对照组60例，两组均接受常规的治疗及护理，治疗组给予口服芪明颗粒，每次1袋，每日3次，对照组给予口服缬沙坦胶囊，每次80mg，每日1次，治疗16周后观察两组患者高敏C反应蛋白（hs-CRP）、白介素-6（IL-6）、白介素-1β（IL-1β）、肿瘤坏死因子-α（TNF-α）及24h尿微量白蛋白（24h UmAlb）的变化。[结果] 治疗组与对照组临床疗效总有效率分别为91.67%、76.67%，比较有统计学差异（P<0.05）。治疗前后两组患者在炎症因子方面均有所改善，但是对照组在hs-CRP、IL-6、IL-1β、TNF-α方面改善无统计学意义（P>0.05），治疗后治疗组在hs-CRP、IL-6、IL-1β、TNF-α方面与对照组比较有统计学差异（P<0.05），两组在24h UmAlb方面比较，各组治疗前后有统计学意义（P<0.05），治疗后比较无统计学意义（P>0.05）。[结论] 芪明颗粒在改善早期糖尿病肾病中医证候、炎症因子方面优于对照组，同时也能明显改善尿微量白蛋白，能够延缓糖尿病肾病的发展。芪明颗粒治疗早期糖尿病肾病安全有效。

Abstract:

[Objective] To explore the mechanism of early diabetic nephropathy by observing the effect of Qiming Granule on the inflammatory factors and urinary albumin in early diabetic nephropathy. [Methods] Selected in accordance with the inclusion criteria of patients with early diabetic nephropathy in 120 cases, using the random number table method 60 patients were divided into treatment group and control group of 60 cases, 2 groups were treated with routine treatment and care, the treatment group to the Qiming Granule 1 bag each time, 3 times a day, the control group was given time a day Valsartan capsules 80 mg on the changes of the two groups were observed in patients with hs-CRP, IL-6, IL-1β, TNF-α, and alpha beta 24 h UmAlb after 16 weeks of treatment. [Results] The total effective rate of the treatment group and the control group were 91.67% and 76.67%, and there were significant differences(P<0.05). The two groups before and after treatment in patients with inflammatory factors have improved, but the control group had no statistical significance in hs-CRP, IL-6, IL-1β, TNF-α aspect (P>0.05), the treatment group in hs-CRP, IL-6, IL-1 β and TNF-α comparison with the control group there were significant differences (P<0.05) in 24 h UmAlb group, the groups were statistically significant (P<0.05) before and after treatment, after treatment was not statistically significant(P>0.05). [Conclusion] Qiming Granule in improving early diabetic nephropathy TCM symptoms, inflammatory factors is better than the control group, but also can significantly improve the urinary albumin, can delay the development of diabetic nephropathy. Qiming Granule is safe and effective in the treatment of early diabetic nephropathy, which provides the experimental basis for the clinical application of Qi Ming Granule.
