李新民教授治疗小儿紫癜性肾炎处方用药探析

摘要:

[目的] 总结归纳李新民教授治疗小儿紫癜性肾炎的组方用药规律和证治思想。[方法] 收集整理在2015年3月-2017年2月在天津中医药大学第一附属医院儿科门诊及住院部经李新民教授诊治的72例紫癜性肾炎患儿的完整处方。运用SPSS 22.0软件对各类处方使用频率超过10%的药物进行聚类分析。[结果] 归纳出李新民教授治疗小儿紫癜性肾炎的4首常用处方：银翘散化裁、不换金正气散化裁、逍遥散化裁、小柴胡汤化裁。分别对各类处方中使用频率超过10%的药物进行聚类分析，得到主要药物组合体4个，4个主要药物组合体与4首处方的高频药物基本吻合。[结论] 总结李新民教授治疗紫癜性肾炎的辨证用药思想发现：在治疗小儿紫癜性肾炎时，主张五脏辨证尤重肺脾，强调卫气营血辨证贯穿其中，治以宣肺疏风，清热凉血；芳香疏化，调理脾胃；疏肝理脾，柔养和营；以及疏解少阳郁热等法，且时时注意调畅气机，宣肺畅肝，运脾醒脾以恢复脏腑生理功能，处方轻巧灵活，有主有次。

Abstract:

[Objective] To summarize the rule of prescription and the thought of syndrome treatment ofallergic purpura nephritis in children treated by professor LI Xinmin.[Methods] The complete prescriptions of 72 patients with allergic purpura nephritis diagnosed and treated by professor LI Xinmin in the outpatient and inpatient department of the first affiliated hospital of Tianjin University of Traditional Chinese Medicine from March 2015 to February 2017 were collected and summarized.[Results] The four commonly used prescription of professor LI Xinmin in the treatment of children with allergic purpura nephritiswere summarized as follows:modified Yin qiao powder,modified Buhuanjin Zhengqi powder,modified Xiao yao powder,modified Xiaochaihu powder. Cluster analysis was carried out on the drugs used more than 10% in various prescriptions,and four major drug combinations were obtained. The four major drug combination were basically consistent with the high-frequency drugs used in the four first prescriptions.[Conclusion] In the treatment of children with purpura nephritis,professor LI Xinmin argue that five viscera syndrome differentiation especially lung and spleen,emphasizes the Weiqi Yingxue syndrome differentiation throughout the camp. With Xuan breeze lung,clear heat cool blood;aromatic hydrophobic,regulate the spleen and stomach;liver and spleen,soft and camp. And relieving shaoyang wind-cold and so on,and always pay attention to the factors,the spleen amuse oneself to restore viscera function,prescription lightweight flexible,have advocate second.
