百令胶囊联合α－酮酸片对慢性肾脏病CKD3-4期患者营养状态及生活质量的影响

摘要:

[目的] 探讨百令胶囊联合α-酮酸片对慢性肾脏病（CKD）3-4期患者营养状态及生活质量的影响。[方法] 选取上海中医药大学附属龙华医院2018年3月-2019年6月收治的90例CKD3-4期患者为研究对象，随机分为对照组、百令胶囊组和试验组各30例。对照组予饮食控制，调节血压、血脂、血糖；改善酸碱失衡及电解质紊乱、纠正贫血等常规治疗，百令胶囊组在对照组基础上加服百令胶囊。试验组在百令胶囊组基础上加服α-酮酸片，持续治疗3个月，比较3组的治疗有效率、肾功能水平、营养不良评分及生活质量评分。[结果] 试验组治疗总有效率显著高于对照组，差异具有统计学意义（P<0.05）；百令胶囊组及试验组治疗后血清肌酐、尿素氮、尿酸均较治疗前降低，eGFR较治疗前升高，组间比较肌酐、肾小球滤过率（eGFR）、血尿酸差异具有统计学意义（P<0.05）。百令胶囊组及试验组治疗后白蛋白、前白蛋白、血红蛋白均较治疗前升高，营养不良评分较治疗前降低，组间比较白蛋白，前白蛋白、血红蛋白、营养不良评分，差异具有统计学意义（P<0.05）。试验组治疗后改善生活质量优于治疗前（P<0.05），差异具有统计学意义；组间比较3组患者治疗后生活质量评分，差异有统计学意义（P<0.05）。[结论] 百令胶囊联合α-酮酸片可有效改善患者肾功能及营养状态水平，提高患者生活质量，延缓疾病进展，值得临床推广。

Abstract:

[Objective] To investigate the effect of Bailing Capsule combined with α-keto acid tablets on nutritional status and quality of life in patients with chronic kidney disease CKD3-4.[Methods] Ninetycases of chronic kidney disease CKD3-4 patients admitted to Longhua Hospital affiliated to Shanghai University of Traditional Chinese Medicine from March 2018 to June 2019 were selected as the research subjects,and randomly divided into control group,Bailing Capsule group and test group,30 cases each. The control group was given dietary control to regulate blood pressure,blood lipids,and blood sugar;to improve acid-base imbalances and electrolyte disorders,to correct anemia,and other conventional treatments,the Bailing Capsule group added Bailing Capsules to the control group. The experimental group added α-keto acid tablets on the basis of the Bailing Capsule group and continued treatment for 3 months. The treatment efficiency,renal function level,malnutrition score and quality of life score of the three groups were compared.[Results] The total effective rate of treatment in the experimental group was significantly higher than that in the control group(P<0.05),the difference was statistically significant;serum creatinine,urea nitrogen,and uric acid in the Bailing Capsule group and the experimental group were lower than before treatment,and eGFR was higher than before treatment High,comparing creatinine,eGFR,blood uric acid(P<0.05),the difference was statistically significant. After treatment,the albumin,prealbumin,and hemoglobin in the Bailing Capsule group and the experimental group were higher than before treatment,and the malnutrition score was lower than before treatment. The albumin,prealbumin,hemoglobin,and malnutrition scores were compared between groups(P<0.05),the difference is statistically significant. The improvement of quality of life after treatment in the experimental group was better than that before treatment(P<0.05),the difference was statistically significant;comparing the quality of life scores of the three groups after treatment between the three groups(P<0.05),the difference was statistically significant.[Conclusion] Bailing Capsules combined with α-keto acid tablets can effectively improve the renal function and nutritional status of patients,improve the quality of life of patients,delay the progression of disease,and are worth clinical promotion.
