摘要:

[目的] 观察快速针刺肩胛下肌激痛点结合传统针刺对肩周炎患者肩关节活动度、疼痛和功能的疗效。[方法] 纳入50例肩周炎患者，应用随机数字表法分为治疗组及对照组，每组各25例，对照组应用传统针刺法进行治疗，治疗组为传统针刺结合针刺肩胛下肌激痛点治疗。每日治疗1次，10次为1个疗程，共治疗2个疗程。观察指标：1）首次治疗后即刻的盂肱关节外旋活动度。2）2个疗程后两组患者盂肱关节外旋活动度、肩关节Constant-Murley评分。[结果] 首次治疗后即刻比较盂肱关节外旋活动度，两组患者均有所改善（P＜0.05）；与对照组比较，治疗组首次治疗后在改善盂肱关节外旋活动度更有优势（P＜0.05）；相比于治疗前，治疗2个疗程后两组患者盂肱关节外旋活动度增大（P＜0.05），Constant-Murley评分提升（P＜0.05），且与对照组比较，治疗组更有优势，差异具有统计学意义（P＜0.05）。[结论]针刺肩胛下肌激痛点结合传统针刺疗法具有增加肩关节外旋活动度、提升肩关节Constant-Murley评分的作用，显现即刻、持久效应，值得临床推广。

Abstract:

[Objective] To evaluate the effect of rapid needling to myofascial trigger points(MTrPs) of subscapular muscle combined acupuncture on the range of motion,pain and function of shoulder joint in patients with scapulohumeral periarthritis.[Methods] The 50 patients with scapulohumeral periarthritis were randomly divided into two groups with 25 cases in each group. The trail concluded with rapid needling of MTrPs combined acupuncture and acupuncutere,which contains two stage. The main observe measures were as follows:1)Degree of glenohumeral external rotation immediately after treatment. 2)Degree of glenohumeral external rotation after two courses of treatment,Constant-Murley score of shoulder joint.[Results] Before treatment,there was no significant difference in glenohumeral external rotation between the two groups and Constant-Murleyt of shoulder (P>0.05). After the first stage, the degree of glenohumeral external rotation was improved in both groups(P<0.05) besides,the improvement of the treatment group was better than that of the control group (P<0.05). The degree of glenohumeral external rotation and Constant-Murley score were improved in both groups after the first treatment,and the improvement of the treatment group was better than that of the control group (P<0.05).[Conclusion] Rapid needling to MTrPs of subscapular muscle combined acupuncture shows the effect of increasing the degree of external rotation of shoulder joint,improving the constant score of shoulder joint,showing immediate and lasting effect.
