基于数据挖掘的针灸及中药熏蒸治疗脑出血偏瘫的药穴规律研究

摘要:

[目的]采用数据挖掘技术分析脑出血偏瘫的针灸治疗选穴规律及中药熏蒸用药规律。[方法]检索中国知网、万方数据知识服务平台、维普数据库自建立至2023年3月10日期间，针灸及中药熏蒸治疗脑出血偏瘫的相关文献，筛选数据并提取有关信息，并运用SPSS Statistics 22.0与SPSS Modeler 18.0进行关联规则及聚类分析，挖掘分析其针灸选穴及用药规律。[结果]足三里、合谷、三阴交为使用频数最高的3个腧穴，手阳明大肠经为使用频数最高的经脉，下肢与上肢为腧穴选择的主要部位，五输穴是使用频数最高的特定穴，关联性最高的腧穴为：足三里→手三里-合谷、足三里→手三里-三阴交、足三里→手三里。聚类分析发现足三里、合谷、环跳、太冲、内关、尺泽、三阴交可以作为治疗主穴选择。中药熏蒸使用频数最高的3味中药分别是红花、伸筋草、当归，药性最多为温性，药味为辛、苦、甘，归肝、脾、肾经。[结论]针灸及中药熏蒸治疗脑出血偏瘫疗效确切，腧穴选择体现同名经配穴、表里经配穴的重要性和以人体解剖为支撑的选穴规律。经络选择要求阴阳结合，且强调“经络所过，主治所及”。聚类分析、关联规则分析为临床治疗提供了主穴及组穴的科学参考。熏蒸用药多选择活血化瘀药、祛风湿药、补虚药。

Abstract:

[Objective] To analyze the rule of acupuncture and moxibustion treatment of hemiplegia in cerebral hemorrhage and the rule of fumigation with traditional Chinese medicine by using data mining technology. [Methods] Literature on acupuncture and traditional Chinese medicine fumigation in the treatment of cerebral hemorrhage hemiplegia from CNKI，Wanfang database and VIP database during the period from March 10，2023， when the database was established，was searched，data was screened and relevant information was extracted. SPSS Statistics 22.0 and SPSS Modeler 18.0 were used for association rules and cluster analysis to analyze acupuncture point selection and drug selection rules. [Results] Zusanli，hegu and sanyinjiao were the three points with the highest frequency of use，the large intestine meridian of shouyangming was the meridian with the highest frequency of use，the lower extremity and upper extremity were the main parts of the selected points，the wushu point was the specific point with the highest frequency of use，and the points with the highest relevance were：zusanli→shousanli-hegu，zusanli → shousanli-sanyinjiao，zusanli → shousanli；cluster analysis showed that zusanli，hegu，huantiao， taichong，neiguan，chize and sanyinjiao could be selected as a group of main treatment points；the three most frequently used acupoints in Chinese medicine fumigation were safflower，sinusia and angelica；many drug properties are warm；medicine taste for hard sweet；return to liver spleen kidney meridian. [Conclusion] Acupuncture and Chinese medicine fumigation are effective in the treatment of cerebral hemorrhage hemiplegia. The selection of meridians requires a combination of yin and yang，emphasizing that “where the meridians pass，there is therapeutic effect”. Cluster analysis and association rule analysis provide scientific references for primary and supplementary acupoints in clinical treatment，matching acupoints with the same name and supporting human anatomy. Fumigation drugs were mainly used to promote blood circulation and remove blood stasis，dispel wind-dampness and tonify deficiency.
