善胃方对萎缩性胃炎癌前病变患者P16蛋白的影响

摘要:

[目的]观察中药善胃方对慢性萎缩性胃炎(CAG)伴有胃黏膜异型增生(Dys)胃和(或)肠上皮化生(IM)的治疗效果,并观察抑癌基因蛋白P16在治疗前后的表达情况。[方法]将内镜和病理确诊为慢性萎缩性胃炎伴胃黏膜异型增生和(或)肠上皮化生(IM)且符合中医气阴两虚型的患者60例,随机分为治疗组30例,给予善胃方;对照组30例,给予猴头菌片,两组均用药6个月。[结果]1)治疗组患者治疗后总有效率为90.00%;对照组患者治疗后总有效率为55.67%,两组有显著性差异P<0.01;2)中药组治疗后多数患者腺体萎缩程度、肠上皮化生、胃黏膜异型增生均有改善;而对照组患者腺体萎缩程度、肠上皮化生、胃黏膜异型增生改善不明显,中药组明显优于对照组,P<0.05;3)中药治疗组治疗前后抑癌基因蛋白P16表达显著提高,P<0.05,而对照组治疗前后P16表达水平无显著变化。[结论]中药善胃方对胃癌前期病变有一定的治疗作用,并通过适当提高P16的表达水平,在一定程度上使胃黏膜病理逆转。

Abstract:

[Objective] To observe the curative effect of Shanwei(improving the structure and function of stomach)on chronic atrophic gastritis(CAG)with gastric mucous membrane dysplsia(Dys)otr intestinal metaplasia(IM)and observe the expression of antioncogene protein P16 before and after treatment.[Methods] The selected patients were suffered from CAG with Dys or IM diagnosed with endoscopy and pathologic examination.Sixty patients were randomly divided into treatment group and control group.The patients of treatment group were administered with Shanwei prescription and patients of control group with heriacium tablets for six months.[Result] The total effective rate of treatment group and control group was individually 90.00% and 55.67%.There was obviously difference between the treatment group and control group(P<0.01).The treatment group had obvious higher effect than control group on improvement in the atrophy extent,intestinal metaplasia and gastric mucous membrane dysplasia(P<0.05).The antioncogene protein P16 expression of treatment group was obviously higher than that of control group(P<0.05).[Conclusion] Sanwei prescription has definitely curative effect on carcinoma of stomach with precancerous lesion and may reverse the pathologic change of gastric mucous membrane on a certain extent by increasing the expression of P16 protein.
