国医大师潘敏求从“郁、毒、虚”论肝硬化代偿期与失代偿期病机及治疗经验

摘要:
肝硬化是临床常见的慢性肝病,病程一般分为代偿期与失代偿期。国医大师潘敏求提出,以“郁”“毒”“虚”贯穿肝硬化全过程:代偿期多以肝郁与湿热瘀毒为主,失代偿期则以脾肾亏虚为本。据此,潘敏求教授制定出“代偿期重在疏郁解毒,失代偿期侧重扶正补虚”的分期辨证治疗体系,疗效显著,为中医治疗肝硬化提供了系统而可行的思路。

Experience of Chinese medicine master PAN Minqiu in treating liver cirrhosis:Pathogenesis and therapeutic approaches of the compensated and decompensate stages from the perspective of “constraint,toxicity and deficiency”

Abstract:
Liver cirrhosis is a common chronic liver disease in clinical practice,typically progressing through two stages:the compensated stage and the decompensate stage. Master of traditional Chinese medicine,Professor PAN Minqiu,proposed that the entire course of liver cirrhosis is characterized by three key pathological factors:“constraint”(Yu),“toxicity”(Du) and “deficiency”(Xu). According to his theory,the compensated stage is primarily marked by liver qi stagnation and damp-heat with blood stasis and toxins,while the decompensate stage is fundamentally due to spleen and kidney deficiency. Based on this understanding,Professor PAN developed a staged and syndrome-differentiated treatment approach:focusing on soothing liver qi and clearing toxins during the compensated stage,and emphasizing strengthening the body’s vital energy and tonifying deficiency during the decompensate stage. This treatment system has demonstrated significant efficacy and offers a systematic and practical approach for the traditional Chinese medicine treatment of liver cirrhosis.
