[目的] 采用中医客观化检测工具脉诊仪对高血压的脉象进行测量,通过对脉图参数与血流变指标的相关性分析,揭示脉图形成原理。[方法] 选取符合纳入标准的原发性高血压患者142例,用脉诊仪采集脉象,同步采集静脉血,用血液流变仪进行血流变指标测试。同时完成中医证候量表的填写。将调查所得资料汇总后,进行统计分析。[结果] 经中医脉图检测及专家判定,高血压脉象以弦脉最多,在75%以上。高血压弦脉脉图参数改变,与全血黏度、血沉、红细胞变形指数、红细胞聚集指数等具有相关关系,且差异有统计学意义(P<0.05).高血压弦脉组实证表现出现频率高,而非弦脉组则以虚证的表现为主。[结论] 高血压脉图分布以弦脉为主。血流变指标与高血压弦脉脉图参数有相关性,可以认为血流变是影响高血压脉图形成与变化的重要相关因素。脉图可以作为高血压中医辨证的依据。 |

Clinical observation of correlation on hypertension between sphygmogram of string-like pulse and blood rheology |

[Objective] Use pulse diagnosis instrument collect sphygmogram of hypertension pulse. By analyzing the correlation between the parameters of the pulse and blood rheology, reveal the formation principle of the pulse. [Methods] Select 142 essential hypertension patients who comply with inclusion criteria. Collect sphygmogram use pulse diagnosis instrument. At the same time, collect blood and test blood rheology by blood rheometer. Then fill in the TCM syndrome scale. Make statistical analysis after gather the data. [Results] According to the traditional Chinese medicine pulse detection and expert judgement, string-like pulse was the most of hypertension pulse, in more than 75%.Parameters of hypertension string-like pulse correlated with whole blood viscosity, erythrocyte sedimentation rate, erythrocyte deformation index, erythrocyte aggregation index, and it was statistically significant (P<0.05). The symptoms of confirm the hypertension string pulse group were the most, while symptoms of asthenia syndrome was the main part in non-string pulse group. [Conclusion] The distribution of hypertension pulse is mainly string-like pulse. Indexes of blood rheology and parameters of hypertension string-like pulse is correlated. It could be considered that blood rheology is the important factor affecting the formation and the change of hypertension. The pulse graph can be used as the basis for TCM syndrome differentiation of hypertension. |
