国医大师卢芳教授从“温督固带”论治强直性脊柱炎经验

摘要:

文章总结国医大师卢芳教授基于《黄帝内经》“阳气者，柔则养筋”理论探讨从“温督固带”治疗强直性脊柱炎的经验。卢教授基于“阳气者，柔则养筋”的中医理论再结合病位，认为强直性脊柱炎的病因病机为督脉“帅”而不统，带脉“枢”而不利，复外感风寒湿邪，形成阳虚不荣则痛，阳郁不通则痛，经络痹阻日久，又添痰瘀互结复杂病机。在治疗强直性脊柱炎时以“温督固带”为总纲，注重温肾阳、通督脉、固带脉兼以散寒除湿、化痰祛瘀，使体内阳气周流恢复，并贯彻“既病防变”思想，在疾病早期即行干预。临证运用自拟方治疗强直性脊柱炎效果明显，为临床治疗强直性脊柱炎提供新思路。

Abstract:

The article summarizes the experience of Professor LU Fang in treating ankylosing spondylitis based on the Yellow Emperor’s Classic of Internal Medicine. Professor LU Fang，a national medical master，based his experience in the treatment of ankylosing spondylitis on the principle of“yang qi is flexible and nourishes the tendons”in the Yellow Emperor’s Classic of Internal Medicine. Based on the Chinese medicine theory of“yang qi，softness nourishes the tendons”and combining with the location of the disease， Professor LU Fang believes that the etiology of ankylosing spondylitis is that the“Shuai”of the directing vessel is not united，and the “Pivot”of the belt vessel is unfavorable，which results in the formation of the“cold and dampness”. External wind，cold and dampness， the formation of yang deficiency does not glorify the pain，yang depression does not pass the pain，meridian paralysis for a long time， and add phlegm and stagnation of each other complex pathology. In the treatment of ankylosing spondylitis，“warming the governor and fixing the belt”should be used to restore the circulation of yang in the body by warming the yang，promoting the flow of qi，removing dampness and dispersing the cold. The effect of using the self-proposed formula in the treatment of ankylosing spondylitis is obvious， which provides a new way of thinking for the treatment of ankylosing spondylitis in the clinic.
