[目的]研究慢性心力衰竭(CHF)患者中医主要证候要素与理化指标之间的相关性。[方法]采用临床流行病学的调查方法,收集CHF患者临床资料,分析统计CHF各证候要素与理化指标相互关系。[结果] CHF主要证候要素组与理化指标之间的相关性分析:CHF与白细胞计数(WBC)、中性粒细胞百分比(NEUT#)、平均血小板体积(MPV)、心肌肌钙蛋白I(cTnI)等呈正相关,与淋巴细胞百分比(LYM#)、单核细胞百分比(MONO#)、红细胞计数(RBC)、血红蛋白含量(HGB)、红细胞平均血红蛋白含量(MCH)、红细胞平均血红蛋白浓度(MCHC)等呈负相关;CHF血瘀证与甘油三酯(TG)呈正相关,与直接胆红素(DBIL)呈负相关;CHF阴虚证与α-羟丁酸脱氢酶(HBDH)呈负相关;CHF阳虚证与LYM#、尿素氮(BUN)、肌酐(Cr)、尿酸(UA)、HBDH等呈正相关,与HGB、MCH、总胆固醇(TC)呈负相关。神经网络分析结果CHF的模型准确性为89.1%;CHF阳虚证的模型准确性为79.3%。[结论] CHF患者的病位不仅仅在心,还可涉及肝、肾等其他脏器及血液系统的一系列变化,这与文献报道相一致。 |

[Objective] To study the correlation between TCM syndrome factors and physical and chemical indexes in patients with chronic heart failure (CHF).[Methods] Using the method of clinical epidemiology,the clinical data of patients with chronic heart failure were collected and analyzed by clinical epidemiology and the correlation between the factors and the physical and chemical indexes of CHF syndrome was analyzed.[Results] CHF had positive correlation with WBC,NEUT#,MPV and cTnI.It had negative correlation with LYM#、MONO#、RBC、HGB、MCH、MCHC.Blood stasis had positive correlation with TG,and negative correlation with DBIL.Yin deficiency had negative correlation with HBDH.Yang deficiency had positive correlation with LYM#,BUN,Cr,UA and HBDH.It had negative correlation with HGB,MCH and TC.Neural network analysis results that the accuracy of CHF model was 89.1%.The accuracy of Yang deficiency model of CHF was 79.3%.[Conclusion] Heart was not only the disease location of CHF patients,but also the liver,kidneys and other organs.This is consistent with the literature. |
